• Illinois OSFM Firefighter II or Illinois OSFM Basic Operations Firefighter Certificate at time of conditional employment offer
• IDPH Paramedic License at time of conditional employment offer PLEASE LIST ANTICIPATED COMPLETION DATE FOR IDPH PARAMEDIC LICENSURE ON THE APPLICATION required documents page.
• CPAT and Ladder Climb verification by date of application deadline (valid if issued 6/24/2021- 6/24/2022)

Sign-in will begin at 8:00 AM CST, please arrive no later than 8:30 AM CST with a photo ID (valid driver’s license, State ID, or Military ID) to sign in and a #2 pencil to take the exam.

An orientation will be provided immediately prior to the administration of the tests to provide further information regarding the testing process. All portions of the testing process are mandatory. Failure to attend and successfully complete any portion of the process will result in elimination from employment consideration.
